The White Practice regrets that it is not at present able to register NHS Patients.


This decision has been taken in order to ensure the practice can continue to safely provide NHS services to its currently registered patients.


If you are not currently registered with an NHS General Practitioner locally, and need to see a doctor or a nurse, you can attend the Walk-in Centre at Ashford.



The practice will register only the following categories:

  • New baby to parents who are already registered with the practice.
  • First degree relative living at same address with a patient who is already registered with our practice.

How to register

The practice will accept applications from patients who are resident in the practice area.

If you wish to register with the practice, please complete the New Patient Questionnaire and hand it to reception with your registration form along with proof of ID (passport or Birth cert.) and proof of your address (up to date utility bill).

You will be registered on our system within 4 to 5 working days from receiving your application.

The questionnaire will allow the practice to know more about you and your general health.

Application forms are available upon request from the reception.

If you have any children under the age of 5, please bring their red book or their immunisation records when you first visit the nurse.
